We understand that the decision to surrender a pet is never easy and is often made only after all other options have been carefully considered. We recognize that for many families, this is a last-resort decision, and our goal is to work with you to find the best possible outcome for both you and your pet. In some cases, we may be able to provide resources or discuss alternatives that could help your pet remain in your home.
Completing this form thoroughly is an important part of the surrender process. The information you provide helps our team understand your pet's personality, medical history, and individual needs so we can make the best decisions for their care and work to find the most appropriate placement.
Please note that submitting this form does not guarantee immediate acceptance of your animal. All animal surrenders are by appointment only and based on available space and resources. We do ask for a $20 donation to cover vaccinations if your pet is not current.
